The current management of pancreas cancer includes identification and dissection of the involved margins with curative intent. The assumption is made that the dissection of pancreas cancer will provide the only definitive treatment for the patient. We examined the role of this convention when comparing a trial of conversion surgery without margin dissection in laparoscopy for biopsy-proven conventional uniportable malignancies of the pancreas. From June 2013 to June 2014, 77 patients with pathologically proven conventional pancreas cancers were treated with laparoscopic ultrasound-guided biopsy via a transgastric approach. The patients were offered surgical conversion to laparotomy and potentially no margin dissection and considered high risk for a margin-free resection with a significant mortality benefit.
